• The victim’s talcum powder use must have occurred after 1982
• The victim must have used Johnson & Johnson’s Baby Powder or Shower-to-Shower for hygiene reasons for four years or more
• The cancer diagnosis must be for fallopian tube or ovarian cancer, not cervical or uterine cancer
• The cancer must have been diagnosed by the time the victim turns 65
• If the victim passes away and her family pursues a claim, ovarian cancer must be listed on the death certificate as the cause of death
• Living victims who file an injury claim must provide ovarian tissue or pathology slides (both should be available if the diagnosis was made within the past 10 years)
